Central Patient Management (ZPM) at the Centre for Surgery

The Central Patient Management (CPM) team is responsible for the central coordination and organisation of surgical planning, admission and bed planning as well as discharge management for patients requiring post-inpatient care. Together with the senior consultants, we plan the operating theatre appointments for all specialist departments for 16 operating theatres and three outpatient operating theatres. In doing so, we take into account the available resources and capacities, such as beds, intensive care and monitoring beds, and pay attention to the urgency of the procedures and special requirements such as Da Vinci, IORT or hybrid operating theatres.

Our nurses in the ZPM work independently and responsibly hand in hand with the doctors in order to optimise the flow of patients within the clinic. Collaboration takes place in an interprofessional and interdisciplinary team - both internally and with external partners - and is based on a broad network.

The team is the point of contact for colleagues, patients, relatives and other professional groups. We are on duty around the clock, even on public holidays and weekends, to ensure the best possible care.

 

Processing an application

The registration for an operation is made either via the various outpatient clinics or consultation hours, directly by the GP or via other clinics. Based on the patient's records and all examination results, the ZPM proposes a date for the operation.
The urgency is taken into account and the availability of bed and operating theatre capacity is clarified.

On the day of admission, we provide information about our care options after discharge.

 

Arrangement of the admission date

We will inform the patient and the family doctor in writing of the scheduled admission date. Comprehensive information about the procedure and the expected length of stay in our clinic will also be provided. As a rule, you should arrive at the surgical clinic 24 hours before the operation.

On the day before admission, we contact the patient by telephone, inform them of the relevant ward and remind them of important points such as examinations and medication.

Admission of a patient

On the day of admission, we ask patients to first report to the information desk at the main entrance. Our staff will be happy to help with luggage and provide further information. From there, the patient will be directed to the inpatient registration desk, to the individual consultations and to the respective ward.

Relatives can of course be present at all consultations.

Linking aftercare

The organisation of follow-up treatment, rehabilitation, cost coverage by health insurance companies and pension insurance providers as well as social law advice is provided by the clinic's social services. Nursing discharge planning, including the organisation of outpatient nursing services, bridging care, stoma therapy and other nursing aids, is carried out by the ZPM team in consultation with the patient, their relatives and colleagues on the ward.
