Social-medical aftercare
Ensuring quality of life in everyday life
The social-medical aftercare at Heidelberg University Hospital supports families with seriously ill children during the transition from inpatient to outpatient care. An interdisciplinary team of paediatric nurses, paediatricians and a qualified social pedagogue support premature and high-risk babies, chronically ill, disabled and ventilated children up to the age of 14 - in special cases up to the age of 18.
The aim is to maintain the quality of life of patients and their families, avoid inpatient stays and stabilise everyday life. The service is free of charge for families and is covered by health insurance on medical prescription.
